经伤椎椎弓根椎体内植骨结合椎板后外侧植骨治疗胸腰椎爆破型骨折的远期疗效分析

摘要: 目的 探讨椎板后外侧植骨与经伤椎椎弓根植骨联合后外侧植骨治疗胸腰椎爆破型骨折的临床远期疗效。方法 2014年1月至2016年11月行后路短节段椎弓根螺钉内固定联合椎板后外侧植骨融合25例(A组);经伤椎椎弓根椎体内植骨联合椎板后外侧植骨融合19例(B组)。所有患者均于术前、术后1周,取内固定前及之后1年行胸腰椎X线及CT检查。结果 术前、术后1周及取内固定前伤椎椎体前缘压缩率及Cobb角2组间比较差异无统计学意义(P>0.05)。取内固定后1年2组间比较椎体前缘压缩率及Cobb角差异有统计学意义(P<0.05)。结论 后路短节段椎弓根螺钉内固定结合经伤椎椎弓根植骨联合椎板后外侧植骨是治疗胸腰椎爆裂型骨折的有效方法。

Abstract: Objective To investigate the clinical long-term effect of posterolateral laminar bone grafting combined with posterolateral bone grafting in the treatment of thoracolumbar burst fractures. Methods From January 2014 to November 2016, 25 cases of posterior short segment pedicle screw fixation combined with posterolateral laminar bone graft fusion were performed in our department(group A). 19 cases were treated with transpedicular bone grafting combined with posterolateral laminar fusion(group B). All patients were underwent thoracolumbar X-ray and CT examination before surgery, 1 week after surgery, before and 1 year after internal fixation. Results There was no significant difference in anterior compression ratio and Cobb angle between the two groups before operation, 1 week after operation and before internal fixation(P>0.05). 1 year after internal fixation, there were significant differences in anterior vertebral compression ratio and Cobb angle between the two groups(P<0.05). Conclusion Posterior short segment pedicle screw fixation combined with transpedicular bone grafting combined with posterolateral laminar bone grafting is an effective method for the treatment of thoracolumbar burst fractures and is worthy of clinical application.
