摘要: 年龄相关性黄斑变性(age-related macular degeneration,AMD)是导致世界范围内老年人群视力减退以及不可逆丧失的重要原因之一。随着人口老龄化进程推进,AMD的发病率增高,一定程度上降低患者自理能力,加重社会及家庭负担。鉴于AMD对患者视力造成的损害是不可逆的,该病现已引起国内外眼科医务工作者的高度重视与关注。脉络膜新生血管生成、萎缩是AMD的特征性表现,在AMD病情进展过程中发挥重要作用,脉络膜新生血管生成、萎缩的发生机制复杂,受多种血管抑制因子与刺激因子的调控。近年来,临床新出现的生物标志物色素上皮衍生因子(pigment epithelium-derived factor,PEDF),是一种血管生成抑制因子,也是丝氨酸蛋白酶抑制剂家族的重要成员,具有神经营养以及调节抗新生血管生成等作用。综述PEDF与AMD的内在关联以及基于PEDF的AMD治疗研究进展,为深入阐释AMD发病机制、探索临床治疗新策略提供理论依据。

Abstract: Age-related macular degeneration(AMD)is one of the major causes leading to vision deterioration and irreversible loss among the elderly population worldwide. As the process of population aging progresses, the incidence of AMD increases, which to some extent reduces the self-care ability of patients and increases the burden on society and families. Given that the damage caused by AMD to patients' vision is irreversible, this disease has now attracted great attention and concern from ophthalmologists at home and abroad. Choroidal neovascularization and atrophy are characteristic manifestations of AMD and play an important role in the progression of the disease. The mechanism of choroidal neovascularization and atrophy are complex and are regulated by various vascular inhibitory and stimulating factors. In recent years, a new clinical biomarker, pigment epithelium-derived factor(PEDF), has emerged. It is a vascular growth inhibitor and an important member of the serine protease inhibitor family, with neurotrophic and anti-neovascularization regulatory functions. The intrinsic retationship between PEDF and AMD, as well as the research progress of AMD treatment based on PEDF, provide a theoretical basis for in-depth elucidation of the pathogenesis of AMD and exploration of new clinical treatment strategies.
