Abstract: Objective To explore the value of out-hospital intensive management in elderly patients with chronic heart failure(CHF).Methods 127 elderly patients with CHF in the Department of Cardiology of Henan Hospital of Traditional Chinese Medicine were selected as the research objects.They were divided into two groups by random drawing.63 cases in the control group were given routine continuity care outside the hospital, and 64 cases in the observation group were combined with intensive management mode outside the hospital.The total rate of improvement of cardiac function, self-monitoring awareness, self-care ability, anxiety and depression mood and re-hospitalization rate were compared between the two groups after treatment.Results After intervention, the self-measured pulse rate, self-measured blood pressure rate and self-weighing rate in the observation group were higher than those in the control group(P<0.05).The levels of self-care maintenance, self-management and self-confidence were significantly better than those of control group(P<0.05).The scores of Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)were decreased significantly than those of control group(P<0.05).Rehospitalization rate(6.25%)was lower than that of control group(19.05%)(P<0.05).The total effective rate of improving cardiac function(95.31%)was higher than that of control group(77.78%)(P<0.05).Conclusion The out-of-hospital intensive management model can effectively improve the self-monitoring awareness and self-care ability of elderly patients with CHF, relieve anxiety and depression, improve the total effective rate of cardiac function improvement, and reduce the re-hospitalization rate.
