足月新生儿窒息后皮质醇的动态变化

摘要: 目的以足月儿血清皮质醇水平作为监测指标,观察不同窒息程度对皮质醇水平变化的影响。方法选择足月儿共90例,其中轻度窒息组28例,重度窒息组32例,对照组为无窒息儿30例。于生后24h内及生后1周时各采静脉血2mL测定血清皮质醇水平。结果(1)足月儿轻度窒息与重度窒息的皮质醇水平在出生后24h内及1周时差异均有统计学意义。(2)足月儿轻度窒息与对照组的皮质醇水平在出生后24h内差异有统计学意义,1周时差异无统计学意义。(3)足月儿重度窒息与对照组的皮质醇水平在出生后24h内及1周时差异均有统计学意义。结论足月新生儿已有完善的肾上腺皮质功能,皮质醇水平与窒息程度相关。

Abstract: Objective To observe the effects of different degree of asphyxia on cortisol levels change in full term infants’ serum cortisol levels as the monitoring indicators. Methods 90 full term infants were selected, 28 cases with mild asphyxia and 32 cases with severe asphyxia, 30 cases with no suffocation as the control group. 2mL venous blood was collected to test the serum cortisol levels within 24 hours and a week after birth. Results (1) Serum cortisol of full-term infants with mild asphyxia and severe asphyxia was obviously different in 24 hours and 7days after birth. (2) The difference of serum cortisol was obvious in 24 hours after birth between group with mild asphyxia and the control group, while it became subtle in a week. (3) The difference of serum cortisol was obvious both in 24 hours and a week after birth between group with severe asphyxia and the control group. Conclusion Full-term infants have already perfect adrenal cortex function, and serum cortisol levels are related to the asphyxia degree.
