糖皮质激素治疗不同原因急性肺损伤研究进展

摘要: 急性肺损伤(acute lung injury, ALI)的临床特征为氧合指数降低、顽固性低氧血症进而发展为急性呼吸窘迫综合征,在危重患者中具有较高的发病率和病死率。其病因多种多样,目前常见为直接肺损伤因素和间接肺损伤因素。每种病因导致ALI的治疗方法不同,尤其是糖皮质激素的应用,且长期使用后有严重并发症,所以使用需谨慎。因此,了解其应用进展及潜在机制有助于更好地应用糖皮质激素,减轻其不良反应。收集和整理关于不同病因ALI使用糖皮质激素的文献,包括动物模型的实验应用以及临床中常用方式,将其研究进展及背后的机制一并进行综述,为高效治疗提供参考。

Abstract: The clinical characteristics of acute lung injury(ALI)are decreased oxygenation index, refractory hypoxemia and then further development of acute respiratory distress syndrome in critically ill patients with high morbidity and mortality. The etiology of ALI is varied, such as direct lung injury and indirect lung injury. Each etiology leads to a different treatment for ALI, especially the use of glucocorticoids. However, the use strategy of ALI is different, and there are serious complications after long-term use, so its use should be cautious. Therefore, understanding the application progress and potential mechanism of glucocorticoids is helpful to better use glucocorticoids and reduce their side effects. This article provides an overview of summary about using glucocorticoids in ALI of different etiology, including the experimental application of animal models and the methods commonly used in clinical practice, and the mechanism behind it, providing reference for efficient treatment.
