保定市医养结合背景下机构养老现况与策略

摘要： 目的对保定市养老机构进行现况调查,了解机构养老的供需现况,提出解决问题的建议,为保定市医养结合模式发展提供决策依据。方法采用定量研究与定性研究相结合的方法。随机抽取保定市13家养老机构中的362位老人,以自行设计的调查问卷为工具,了解其医疗与养老需求;通过访谈养老机构负责人,了解老年人养老和医疗需求供给状况。结果保定市养老机构入住老人调查结果中,老年人对于日常照料需求中的“护理人员协助床椅/床与床转移”的需求最强烈(45.58%),对“护理人员协助进食”需求最弱(8.01%);医疗保健需求中,老人对“护理人员定期测量血压”的需求度最高,为55.52%;在心理需求中,高达95.58%的老人表达了希望儿女来探望,而对此条目的满意度却较低,为74.27%。在养老机构供给状况中,受访的13家养老机构中获得医疗经营许可的只有3家(23.08%);在医疗卫生服务内容上,53.84%的养老机构可提供“常规体检”和“健康咨询”服务,38.46%可提供“医疗诊断”,而能够提供“康复治疗”和“临终关怀”的机构最少,只占30.76%。结论保定市养老机构需要加快建立医养机构合作机制、努力建设优质人才队伍。

Abstract: Objective A cross-sectional study was conducted on the elderly institutions in Baoding to understand the current situation of the supply and demand of institutional pension and put forward suggestions for solving the problemsso as to provide decision-making basis for the development of medical-nursing combined care mode in Baoding. Methods The method of combining quantitative and qualitative research was adopted. A total of 362 elderly people in 13 nursing homes in Baoding were randomly selected to understand their medical and pension needs by self-designed questionnaire. Through in-depth interview, we interviewed the heads of nursing institutions, and found out their supply situation and difficulties. Results In the result of the survey of the elder in nursing homes in Baoding, the elderly had the strongest demand for ‘nursing assisting bed chair/bed and bed transfer’(45.58%), and the demand for ‘nurses to help eat’ was the weakest(8.01%), in the medical care needs of the elderly, ‘nursing staff of regular blood pressure measurement’ was the highest, was 55.52%, in the psychological demand, up to 95.58% of the people expressed the hope that the children came to visit, and the purpose of this satisfaction was reduced to 74.27%. In the supply of aged care institutions, access to medical licenses of 13 pension institutions surveyed in only 3(23.08%), in the medical and health services, 53.84% aged care institutions could provide a ‘routine physical examination’ and ‘health advisory service’, 38.46% could provide ‘medical diagnosis’, and could provide ‘rehabilitation therapy’ and ‘deathbed care’ institutions at least, accounted for only 30.76%. Conclusion Facing the difficulties and problems of the aged care institutions in Baoding, it is necessary to speed up the establishment of cooperation mechanism of medical and nursing institutions and strive to build a team of high-quality talents and so on. The city of Baoding needs more exploration and effort, so that the medical-nursing combined care mode can gain long-term development.
