极低出生体质量患儿出生早期血清Nesfatin-1表达水平变化

摘要: 目的 探究血清Nesfatin-1在极低出生体质量患儿中的表达及临床意义。方法 选取2020年10月至2024年8月在河北大学附属医院出生并于出生后即转入新生儿重症监护室住院的89例极低出生体质量患儿作为研究对象,依据出生胎龄分为3组,A组(胎龄<28周)10例,B组(胎龄28~30周)20例,C组(胎龄>30周)59例,检测并观察所有患儿出生早期血清Nesfatin-1水平变化规律。结果 胎龄<28周 患儿出生第1 天、7 天、14 天、1个月及3个月血清Nesfatin-1水平显著高于 胎龄28~30周和胎龄 >30周,差异有统计学意义(P<0.05);且随着胎龄的增长,血清Nesfatin-1水平呈逐渐下降趋势;各个胎龄时间段血清Nesfatin-1在出生第1 天表达水平均最高,但在出生的前7 d呈下降趋势,随后逐渐升高,出生14 d后随着出生日龄的延长,血清Nesfatin-1水平呈下降趋势,差异有统计学意义(P<0.05);胎龄<28周 患儿体质量增长速率、身长增长速率及头围增长速率均显著低于 胎龄28~30周和胎龄>30周,差异有统计学意义(P<0.05);高水平血清Nesfatin-1与新生儿体质量增长速率、身长增长速率及头围增长速率呈负相关(r分别为-0.652、-0.503、-0.589,均P<0.05)。结论 极低出生体质量患儿出生早期血清Nesfatin-1水平存在动态变化,且与体格生长发育存在一定相关性,临床上可将其作为极低出生体质量患儿生长管理的依据。

Abstract: Objective To explore the expression and clinical significance of serum Nesfatin-1 in children with very low birth body mass.Methods 89 cases of children with very low body quality born in and after birth transferred into neonatal intensive care unit were selected from October 2020 to August 2024 in Affiliated Hospital of Hebei University as the study object. According to birth gestational age, they were divided into 3 groups, group A(gestational age <28 weeks, 10 cases), group B(gestational age 28-30 weeks, 20 cases), group C(gestational age >30 weeks, 59 cases), early serum Nesfatin-1 level changes of all children were detected and examinated.Results Children with gestational age <28 weeks had significantly higher serum Nesfatin-1 levels on days 1, 7, 14, 1 month and 3 months than that of gestational age 28 to 30 weeks and gestational age >30 weeks, the difference was statistically significant(P<0.05). With increasing gestational age, serum Nesfatin-1 level showed a gradual decline. The expression level of serum Nesfatin-1 was highest at day 1 of birth for each gestational age period, but showed a decreasing trend during the first 7 days of life, following a gradual elevation, with the increasing age of birth after 14 days of life, falling trend in serum Nesfatin-1 levels, the difference was statistically significant(P<0.05). The growth rate of body mass, body length and head circumference of children with gestational age <28 weeks were significantly lower than that of the gestational age of 28 to 30 weeks and the gestational age of >30 weeks, the difference was statistically significant(P<0.05). High levels of serum Nesfatin-1 were negatively associated with the growth rate of neonatal body mass, body length and head circumference(r was -0.652, -0.503 and -0.589, respectively, P for all <0.05).Conclusion The serum Nesfatin-1 level changes dynamically in the early birth, and it has some correlation with the physical growth and development, which can be used clinically as the basis for the growth management of children with very low birth body quality.
