综合叙事护理评估在安宁疗护病房癌症患者照护中的应用

摘要: 目的 运用综合叙事护理评估方法,使用奥马哈问题分类系统对安宁疗护病房的癌症患者进行评估,发现患者的主要照护问题,帮助安宁疗护团队为患者及家属提供后续的安宁疗护服务提供实践依据,以改善患者生命末期生存质量,同时提升安宁疗护团队的照护能力。方法 本研究采用便利抽样法选取首都医科大学附属北京潞河医院安宁疗护病房300例癌症患者,运用综合叙事护理评估方法进行评估,采用改良后的奥马哈问题分类系统收集患者的照护问题。结果 300例患者的主要照护问题依次为精神健康、消化-水合、灵性、疼痛、睡眠和休息型态、角色改变等。结论 综合叙事护理评估结合奥马哈问题分类系统有助于安宁疗护团队全面、有效地发现癌症患者的照护问题,从而为患者及家属提供更优质的安宁疗护服务。

Abstract: Objective To evaluate cancer patients in the palliative care ward by applying the comprehensive narrative nursing assessment method and the Omaha Problem Classification System, the main care problems of the patients were identified, providing practical basis for the palliative care team to offer subsequent palliative care services to patients and their families, in order to improve the quality of life of patients at the end of their lives and enhance the care capabilities of the palliative care team. Methods In this study, 300 cancer patients from the palliative care ward of Beijing Luhe Hospital Attached to Capital Medical University were selected by convenience sampling. The comprehensive narrative nursing assessment method was used for evaluation, and the modified Omaha Problem Classification system was adopted to collect the care problems of the patients. Results The main nursing problems of the 300 cases were mental health, digestion and hydration, spirituality, pain, sleep and rest patterns, role change, etc. Conclusion Comprehensive narrative nursing assessment combined with the Omaha Problem Classification System helps hospice care teams to identify care issues for cancer patients comprehensively and effectively, thereby providing better hospice care services for patients and their families.
