Abstract: China is one of the countries where the prevalence of diabetes is increasing rapidly in the world, and it is also the country with the largest number of diabetes patients. Diabetes health management is a key measure for controlling blood sugar, improving patient compliance, and effectively reducing chronic complications. Domestically, various diabetes management models have been continuously innovated and constructed, including hospital-centered models, community-centered models, and hospital-community integrated models, etc. This article aims to further summarize the advanced experiences in diabetes management both at home and abroad, actively explore suitable diabetes management models for this region, and provide theoretical references for formulating diabetes management models that are suitable for this region.
