Abstract

Abstract: Transitional care for hospice patients plays a vital role in improving the continuity of hospice services and enhancing the quality of life for patients and their families. This paper focuses on the transitional care of hospice patients and their families during transitions between hospitals, communities, and homes, and analyzes the existing problems in hospice transitional care. It further proposes several targeted strategies, including the use of standardized communication templates and techniques, implementing training programs to enhance awareness and practical skills in hospice care, providing supportive care services, developing tailored transitional care intervention plans, and establishing a tiered hospice care referral system.This paper aims to provide a reference for improving the quality of hospice transitional care in China.
