Effect of different health education methods on fall vigilance of elderly inpatients

摘要: 目的 探讨常规健康教育、基于收益信息框架的健康教育及基于损失信息框架的健康教育在住院老年患者预防跌倒中的应用效果。方法 选取2025年7—8月在保定市某三甲医院神经内科及康复病房住院的96例老年患者为研究对象。将96例研究对象分为3组,对照组实施常规健康教育。为进一步明确收益框架和损失框架的影响,将干预组分为收益信息框架组和损失信息框架组。收益信息框架组在常规健康教育基础上增加收益信息框架理论指导的健康教育;损失信息框架组在常规健康教育基础上结合损失信息框架理论干预。比较3组患者干预前后的跌倒警觉度及跌倒自我效能得分情况。干预期间对照组失访2例,收益信息框架组失访1例。结果 3组患者干预后跌倒警觉度均较干预前提升(P<0.05),其中收益信息框架组与损失信息框架组的跌倒警觉度及跌倒自我效能得分均高于对照组,差异有统计学意义(P<0.05),损失信息框架组在提升跌倒警觉度和自我效能方面优于收益信息框架组,差异有统计学意义(P<0.05)。结论 基于信息框架效应理论设计的健康教育方案有效增强老年住院患者的防跌倒意识与自我管理能力,且损失信息框架导向的干预策略更具优势。

Abstract: Objective To explore the application effect of routine education, gain-based framework healtheducation and loss-based framework health education in fall prevention of hospitalized elderly patients. Methods Ninety-six elderly patients who were hospitalized in the Department of Neurology and Rehabilitation Ward of a tertiary hospital in Baoding city from July to August 2025 were selected as study subjects. The subjects were divided into three groups: the control group implemented conventional health education; to further clarify the impacts of the gain framework and loss framework, the intervention group was divided into the gain-framing group and the loss-framing group. The gain framework group added gain framework theory-guided health education on a conventional basis; and the loss framework group combined loss framework theory intervention on a conventional basis, comparing the fall alertness and fall self-efficacy scores of patients in the three groups before and after the intervention. There were 2 cases of loss of visits in the control group and 1 case of loss of visits in the loss framework group during the intervention period. Results Falls alertness was significantly increased in all three groups of patients after the intervention compared with the pre-intervention period(P<0.05), in which the alertness and falls self-efficacy scores were higher in the gain-framing group and loss-framing group than in the control group, with statistically significant differences(P<0.05), and the loss-framing group was better than that of the gain-framing group in improving falls alertness and self-efficacy, with statistically significant differences(P<0.05). Conclusion The health education program designed based on the theory of information frame effect can effectively enhance the fall prevention awareness and self-management ability of elderly inpatients, and the loss frame-oriented intervention strategy is more advantageous.
