糖尿病管理模式的研究进展

摘要/Abstract

摘要： 中国是全球糖尿病患病率增长最迅猛的国家之一,同时,也是糖尿病患者数量最多的国家。糖尿病的健康管理是控制血糖、提高患者依从性以及有效减少慢性并发症的关键举措。国内不断创新构建多种糖尿病管理模式,包括以医院为中心的管理模式、以社区为中心的管理模式、医院-社区一体化的管理模式等。总结国内外糖尿病管理的先进经验,积极探索适合本地区的糖尿病管理模式,为制定符合本地区的糖尿病管理模式提供理论参考。

关键词: 糖尿病, 管理模式, 医院-社区一体化

Abstract: China is one of the countries where the prevalence of diabetes is increasing rapidly in the world, and it is also the country with the largest number of diabetes patients. Diabetes health management is a key measure for controlling blood sugar, improving patient compliance, and effectively reducing chronic complications. Domestically, various diabetes management models have been continuously innovated and constructed, including hospital-centered models, community-centered models, and hospital-community integrated models, etc. This article aims to further summarize the advanced experiences in diabetes management both at home and abroad, actively explore suitable diabetes management models for this region, and provide theoretical references for formulating diabetes management models that are suitable for this region.
