The effects of telephone follow-up continuity of care on non-medicine measure of secondary prevention of patients with coronary heart disease

Abstract

Abstract: Objective To probe into influence of telephone follow-up continuity of care on non-medicine measure of secondary prevention of patients with coronary heart disease.Methods 68 patients with coronary heart disease after discharge were regular followed up and carried out health education intervention.Life style score was investigated before and after intervention,including diet,no smoking,physical exercise and emotional control.Results Life style score was changed significantly except emotional control.Conclusion Telephone follow-up continuity of care on non-medicine measure of secondary prevention can improve life style.The intervention of emotional control should be strengthened.
