    Effects of Xuefu Zhuyu Decoction on MMP-2 and TIMP-2 in atherosclerotic rats

    Objective To explore mechanism of Xuefu Zhuyu Decoction on the expression of matrix metalloproteianse-2(MMP-2) and tissue metalloproteinase inhibitor -2 (TIMP- 2) in atheroselerotic rats. Methods 30 SD rats were randomly divided into the control group, model group and Xuefu Zhuyu Decoction group. Subcutaneous injection vitD3 with high fat feeding was conducted to reproduce atheroselerotic rats model. After the experiment, immunohistochemistry staining was performed to detect the level of MMP- 2 and TIMP-2 protein expression. Results Under light microscope, arterial structure of the control group was completed, artery intima of the model group was rough, the smooth muscle was disordered arrangement, Xuefu Zhuyu Decoction forms was between the two;Compared with the control group, the expressions of MMP-2 was increased signiifcantly and the expressions of TIMP-2 was decreased signiifcantly in in model group and Xuefu Zhuyu Decoction group. Compared with the model group, the expressions of MMP-2 protein in Xuefu Zhuyu Decoction group were decreased signiifcantly, the expressions of TIMP-2 protein were increased signiifcantly.Conclusion The Xuefu Zhuyu Decoction Could downregulate the expressions of MMP-2 and upregulate he expressions of TIMP-2 and inhibit the formation of atherosclerosis.

    Objective To research the inhibition of human gastric cancer SGC-7901 cells and drug-resistance cell strain in vitro by Diosbulbin B. Methods Induce human gastric cancer cell line SGC-7901 by drug concentration incremental method and set up its drug-resistance cell line of cis-platinum, 5-FU and adriamycin amycin respectively. Test the proliferation inhibition ratio of Diosbulbin B to SGC-7901, SGC-7901/cis-platinum SGC-7901/5-FU and SGC-7901/adriamycin amycin applying MTT method, at the meantime,compare the proliferation inhibition ratio between SGC-7901 and its drug-resistance cell strain. Results Diosbulbin B showed an effect of concentration dependent anti-proliferation within the scope of certainly concentration range to human gastric cancer cell line SGC-7901, SGC-7901/cis-platinum, SGC-7901/5-FU and SGC-7901/adriamycin (P<0.05), meanwhile, Diosbulbin B showed same effect within the scope of certainly concentration range to cis-platinum, 5-FU and adriamycin amycin drug-resistance cell strain of human gastric cancer cell line SGC-7901 (P>0.05). Conclusion Diosbulbin B show low cross resistance on the human gastric cancer cell line SGC-7901, and develop the same affect of proliferation inhibition when the cell line multidrug resistance with cis-platinum, 5-FU and adriamycin amycin.

    Objective To investigate the effects of spinal anesthesia for painless labor on the short-term and long-term social adaptability of newborn. Methods 221 full-term pregnant women with vaginal delivery were included in our study. Patients were allocated to receive spinal anesthesia for painless labor (n=105, painless labor group)or no analgesia labor (n=116, non painless labor group). The neonatal Apgar score and umbilical artery blood pH detection were done in all newborns. And they were followed up when they were 7 years old. Adaptive behavior scale were obtained and compared. Results There were no signiifcant differences in neonatal Apgar score and umbilical artery blood pH between the two groups (P>0.05). Total scores of Adaptive Behavior Scale were 192.67±13.53 in painless labor group, and 196.31±14.32 in non painless labor group, there was no significant differences between the two groups (P>0.05). Conclusion Spinal anesthesia for painless labor has no significant effect on neonatal short-term and long-term social adaptation ability, which can be widely distributed in clinical practice.

    Objective To study the clinical application value of Myocardial Enzyme in the cardiac damage of patients with Rheumatoid Arthritis(RA) and provide a reference basis for the prediction of diagnosis and treatment of RA with cardiac damage. Methods The serum of 89 patients of RA between 2010 and 2013 were collected and detected the level of CK and CK-MB in serum with machine. They were divided into two groups, one group was patients of cardiac damage with RA and the other group was no cardiac damage. The healthy group is from the regular health check-up people. Results The incidence of RA with cardiac damage was 37.1%. The level of CK and CK-MB in cardiac damage with RA was signiifcantly higher than the other two groups, There was signiifcant differences in comparison between the group of cardiac damage with RA and the other two groups to the level of CK and CK-MB(P<0.05). There was no signiifcant differences in comparison between the group of no cardiac damage with RA and the healthy group to the level of CK and CK-MB (P<0.05). Conclusion There is important value in predicting cardiac damage to detect the level of Myocardial Enzyme in patients of RA and provide a reference basis for the prediction of diagnosis and treatment of RA with cardiac damage, which deserves further clinical application.

    Objective To investigate the related risk factors of myocardial enzyme changes in patients with cerebral hemorrhage. Methods 125 patients with acute cerebral hemorrhage, according to the amount of bleeding, the bleeding site and Glasgow score were studied retrospectively, all patients were divided into corresponding groups, compared with 50 health physical examination cases, the changes of myocardial enzyme in acute stage were observed. Meanwhile myocardial enzyme changes were monitored to acquaint the enzyme change trend. Results Compared with healthy control group, the patients‘amount of bleeding in the experimental group was less than 20 mL, Glasgow score was more than 12, which had not a signiifcant difference in myocardial enzyme (P>0.05), the patient’s amount of bleeding was more than 20 mL, Glasgow score less than 12 , which had a significant difference in myocardial enzyme (P<0.05), Myocardial enzyme changes were associated with the bleeding site. Thalamic hemorrhage had the most significant changes in myocardial enzymes. Myocardial enzyme reached peak at 72 hours in the patients with cerebral hemorrhage,14 days returned to normal level. Conclusion The changes of myocardial enzyme in patients with cerebral hemorrhage is closely related with the amount of bleeding, the bleeding site and the severity of the disease, which can be used as an effective indictor to monitor disease progression and prognosis.

    Objective To evaluate the efficacy of transforaminal lumbar interbody fusion combining contralateral percutaneous pedicle screw for degenerative lumbar disc herniation by Wiltse approach. Methods We studied 40 patients affected by lumbar disc herniation underwent transforaminal lumbar interbody fusion combining contralateral percutaneous pedicle screw ifxation by Wiltse approach in our hospital from June 2009 until December 2012 retrospectively. Clinical and radiographic evaluation were carried out preoperatively and following surgery. Clinical outcome was assessed using the Visual Analog Scale scores (VAS) and The Japanese Orthopaedic Association (JOA) scores. Results The average follow-up time was fourteen months, the average blood loss was three hundreds twenty-six milliliter, the difference of postoperative VAS score and JOA score comparing with preoperative was statistically significant(P<0.01). All screws showed very good position by photos. Bone fusion was considered as solid in forty patients at postoperative 12 months (100%). Conclusion Transforaminal lumbar interbody fusion using percutaneous pedicle screw ifxation by Wiltse approach is a valid and safe treatment of degenerative lumbar disc herniation.

    Objective To invsetigate the effects of Tirofiban Hydrochloride on platelet aggregation rate and major adverse cardiovascular events (MACE) in patients with non-ST-segment elevation myocardial infarction. Methods 99 patients who were diagnosed as NSTE-AMI were randomly divided into Tiroifban Hydrochloride group (group A, 50 cases) and the routine treatment group (group B, 49 cases). Group A was treated with Tiroifban Hydrochloride, administered as a dose of 0.4 μg/(kg · min) over a 30-minute period and followed by a 72-hour infusion of 0.1 μg/(kg· min), in addition to routine treatment. as group B was treated with routine treatment. Platelet aggregation rates were measured before treatment and after 72-hour treatment. Patients were followed up for 30 d, the incidence of major adverse cardiovascular events and the adverse reactions were observed. Results The platelet aggregation rate levels were decreased more obviously in group A after treatment for 72 hours (P<0.05). After 30 days, the incidence of major adverse cardiovascular events was lower in group A than that in group B(P<0.05). There was no severe haemorrhage in both groups. Conclusion Early use of Tirofiban Hydrochloride can reduce the platelet aggregation rate obveriously in patients with non-ST-segment elevation myocardial infarction, which can reduce the incidence of major adverse cardiovascular events. Tiroifban Hydrochloride is effective and safe in clinical practice.

    Objective To explore the partial function of the macular area before and after idiopathic macular hole surgery with multifocal ERG (mfERG). Methods Macular hole standard three-line incision vitreous stage III and IV patients with idiopathic cutting-linking and 0.05%indocyanine green stained internal limiting membrane peel plus 20%SF6 gas-iflled treatment, preoperative and postoperative line mfERG were used in the study. Results 1 and 2-ring surgery in patients with a ifrst-order reaction function of P1 wave density were signiifcantly reduced compared with normal eyes. 1 and 2-ring P1 response densities macular hole healing after eye surgery mfERG first-order function of the gradual increased, central peak postoperative mfERG three-dimensional topographic map of gradual recovered, but still lowered than the normal eyes after 6 months. Conclusion mfERG can accurately relfect the local function of the macular area.

    Objective To observe the rehabilitation of early intervention impact on the shoulder subluxation after stroke hemiplegic. Methods Using the method of randomization, patients were divided into observation group and control group, 30 cases in each. Both groups received early neurological conventional drug therapy and acupuncture, functional electrical stimulation, observation group basing on this were also given early rehabilitation guidance timely. Results After a month of shoulder protection of treatment, 18 cases were effective (60.0%), 9 cases of observation group were improved, 3 cases were failed, and that of the control group was 33.3%(10/30). There was a signiifcant difference between the 2 groups(P<0.05). Conclusion Early rehabilitation in conjunction with acupuncture, functional electrical stimulation fundamentally can reduce the incidence of shoulder subluxation after stroke.

    Objective To understand the impact of old age pregnancy on mother and infant. Methods 4 021 pregnant women with childbirth from January 1, 2012 to December 31, 2012, were divided into old age group (Observer Group) and senior group (control group), the pregnancy complications and caesarean section rates and postpartum hemorrhage rates of the 2 groups were analysed. Results The pregnancy-related complications, cesarean section rates and postpartum hemorrhage incidence rates of observer Group were signiifcantly higher than that of the control group. There was a signiifcant differences between the 2 groups (P<0.05). Conclusion The results indicate that we should pay attention to the elderly pregnancy on maternal and perinatal, strengthen prenatal and intrapartum care to ensure the safety of maternal and child health.

    Objective To observe the effect of Jingju liquid medicine for external use for infantile eczema. Methods 123 cases of infantile eczema were randomly divided into therapy group and control group. The therapy group was treated externally with Jingju liquid medicine, and the control group was treated externally with vitamin B6, then to observe the effect within 7 days as a course of treatment for both groups. Results The effective rate of therapy group was 96.83%, the effective rate of the controls was 83.33%, there was a signiifcant difference between the 2 groups (P<0.05). The curative days of therapy group was (8.63±2.25) days in average, while the curative days of control group was (10.08±3.91) days in average, the former was remarkably less (P<0.05). Conclusion Jingju liquid medicine for external use is a simple, safe prescription, with good curative effects and convenience, which is worthy to be widely distributed in the clinical practices.

    Objective To investigate the affected inlfammatory cytokines of preemptive analgesia with parecoxib sodium on the patients with hip replacement, and observe the postoperative analgesia. Methods 60 cases aged 40 to 65 years, ASAⅠ-Ⅱgrade, under general anesthesia for elective hip arthroplasty were randomly divided into observation group and control group, 30 cases in each group. Before general anesthesia for 20 min, the patients of observation group were injected parecoxib sodium 40 mg (diluted with saline to 4 mL) intravenously, the patients of control group were given saline 4 mL intravenously. Before bolus parecoxib sodium or saline 10 min (T1), end of the surgery (T2), after 6 h (T3), Postoperative 12 h (T4), Postoperative 24 h (T5) Exsanguinate vein blood samples and detecting the inlfammatory cytokine interleukin-6 (IL-6), tumor necrosis factor-α(TNF-α) in the plasma concentrations respectively. Using a visual analogue scale (VAS) evaluation of surgery immediately, after operation for 2 h, 6 h, 12 h, 24 h analgesic effects were observed . Results Inlfammatory cytokines:The IL-6 plasma concentration of the two groups were signiifcantly higher at the time point T3,T4 and T5 than pre-operation. There was statistical difference between the 2 groups(P<0.05). The IL-6 plasma concentration of the experimental group at the time point T3,T4 and T5 was obviously lower than that of the control group(P<0.05). The TNF-αplasma concentration of the two groups were signiifcantly higher at the time point T4 and T5 than pre-operation (P<0.05). The TNF-αplasma concentration of the experimental group at the time point T4 and T5 were obviously lower than that of the control group and there was statistical difference(P<0.05). Stress hormone concentration:The plasma concentration of the two groups at the different time point were clearly higher than that before operation and there was statistical difference(P<0.05). The plasma concentration of the experimental group at the different time points T2,T3, T4 and T5 were obviously lower than that of the control group. The plasma E concentration of the two groups at the time point T2 were obviously higher than that before operation. The plasma E concentration of the experimental group at the time point T2 were obviously lower than that of the control group and there was statistical difference(P<0.05). The evaluation result of VAS: The VAS evaluation results of the experimental group at the different time points---the very end of the operation, 2h, 6h,12h after the operation, were significantly lower than that of the control group and there was a significant difference between the 2 groups (P<0.05). Conclusion Parecoxib sodium preemptive analgesia can relieve postoperative pain in total hip arthroplasty patients, inhibit the release of inlfammatory mediators, reduce the sensitivity of the nervous system, which will help the rehabilitation of patients after surgery.

    Objective To investigate the diagnostic mothods and key-point of hamartoma of lung. Methods 8 gamartoma of lung cases form Dec. 2008 to Dec. 2012 of our hospital were enrolled in this retrospective study, and the X-ray and CT image dates were analysed. Results All of the cases were peripheral, 6 cases (75%) were at pulmo dexter, anyd 2 cases (25%) were at pulmo sinister. The hamartomaic diameter were 1-7 cm, and the average was 3.0 cm. The tumor morphology were round or oval, 3 cases`(37.5%) contours were clear and edges were smooth, 5 cases`(62.5%) contours were shallow lobulated and edges were sharp. 6 cases (75%) were homogeneous in density, and 2 cases (25%) density were unhomogeneous. 1 of the cases exhibited the“popcorn”calciifcation, 1 of the cases exhibited the punctiform calciifcation. CT pathology morphology:2 cases`tumor morphology were round or oval, contours were clear and edges were smooth, 6 cases`contours were lobulated, there was no satellite opacities, the accumulation of blood vessels and pleural indentation in the whole cases.5 cases were homogeneous in density, and 3 cases density were unhomogeneous. 1 of the cases exhibited the“popcorn”calciifcation, 1 of the cases exhibited the punctiform and cords calciifcation, and partly low density liparoid. CT evaluation of enhanced:3 cases`reinforcement value<10 HU, 4 cases`reinforcement value were between 10-20 HU, and 1 case reinforcement value >40 HU. Conclusion CT pathology morphology has a signiifcant meaning in the identiifcation of pulmonary hamartoma, especially the evaluation of the lipids parts, when the lipids island appears in the nidus, and the CT result represent the low density liparoid, thus can be regard as a speciifc signal of pulmonary hamartoma.
    Relationship between iodine content in drinking water and well depth in inland iodine deifcient area

    Objective To explore the relationship between iodine content in drinking water and well depth in iodine deifciency area, and provide scientiifc basis for the development of prevention strategies to control iodine deifciency disorders. Methods According to the“Hebei Province resident drinking water iodine content survey program”, to carry water iodine monitoring for the residents of Yi County, water iodine contents were determined through arsenic cerium catalytic spectrophotometric method. Correlation analysis was carried out on the relationship between iodine content in drinking water and well depth. Results The water iodine average value of well depth was less than 20.00 m was 2.00μg/L or less. The water iodine average value of well depth between 20.00-180.00 m was 2.00μg/L or more. Iodine average value of different groups well depth was less than 10μg/L. By the correlation analysis, water iodine content and well depth existed the weak correlation in mountainous area(r=0.165, P<0.05), this situation had no true signiifcance(r2=0.027). There was no signiifcant correlation between water iodine content with well depth in hills and plain(r=-0.078, -0.076, P>0.05). Conclusion Water iodine content were very low, regardless of deep well or shallow well, and water iodine content has no correlation with well depth in existing wells in Yi County.

    Objective To analyze the root cause of nursing adverse events and formulate the corresponding measures. Methods The status of 49 nursing adverse events were analyzed retrospectively, the primary causes were traced to enable control measures to be devised. Results Accidents which including bed fall, tumble and empyrosis, unplanned extubation, infusion reaction, extravasation of drugs were the top four rankings of nursing adverse events. In the departments including general surgery, Neonatology, ICU, Obstetric, the incidence of adverse events was higer than that of other departments. The nurses with less than 5 years of service were predominant in the nurse responsible for the adverse events, accounting for 66.4%. Conclusion It shoud be strengthened to train the junior nurse and to manage variety catheters, the risk assessment of a high-risk group should be carried out to reduce the occurrence of accidents and to ensure the patients safety.

    Human sperm DNA damage is one of the common causes of reducing male reproductive capacity. A number of factors can bring out the DNA strand breaks or make double-stranded DNA into single-stranded DNA. To a certain extent, some sperm with damaged DNA may escape from the sperm surveillance mechanism and transmit the damage to the offspring, which may give rise to the associated diseases such as abnormal embryonic development, early natural abortion, congenital anomalies and so on. This review assessses the damage factors of sperm DNA, which include ages, time of celibation, environment, freezing of sperm, oxidative stresse, minor element and the abnormal assembling of chromatin, which provides some basic information for elucidating the mechanism of DNA damage, prevention and treatment of male infertility as well as optimal spermatozoa selection in assisted reproductive technique.

    Coronary heart disease (CHD) angina pectoris is a common clinical disease with high rates of incidence, morbidity and mortality, which has a serious inlfuence on human health, especially on the mental workers aged more than 40-year-old who are in high risk. Many Chinese medical scholars were committed to the research of prevention and treatment for CHD angina pectoris all along, and they developed a number of new Chinese medicine. In this paper, the clinical research data of blood stasis CHD angina pectoris were selected, and it emphatically summarized the clinical drug efifcacy of blood stasis type of angina pectoris syndrome in recent years, as well as the research and development (R&D) status of new Chinese medicine.

    Alzheimer's disease (AD) is a fatal neurodegenerative brain disease. The senile plaques formed by Aβamyloids’ sedimentary in extracellular space and intracellular neuroifbrillary tangles(NFTs) formed by excessive phosphate Tau protein aggregation are the main characteristic lesions. The abnormal Tau protein is closely related to the neurodegeneration and the decline of cognitive abnormalities of AD. The interaction between Tau protein and related molecules has further promoted the occurrence and development of AD. This paper reviews the interactions and relationships between Tau protein and Aβ, 14-3-3ζ, p62 in AD.

    Endothelins (ETs) are composed of 21-aminoacids produced by endothelial cells and considered to be potent vasoconstrictor. Recent studies found that they could not only induce the angiogenesis, mitosis and differentiation, but also act the role of cytokines. Previous studies have identiifed three isoforms of ET (ET-1, ET-2 and ET-3), which are involved in a variety of physiological and pathological processes. ET-1 is most potent one among ETs, whereas ET-3 is the weakest once. ET peptides exhibit their functions in a number of mammalian physiological systems, including gastrointestinal. ETs act via two distinct isoforms of receptors in human body, i.e. endothelin receptor A (ETA) and endothelin receptor B (ETB), which can be found expressed in many mammalian cardiovascular system, neural system and gastrointestinal tract. ETA and ETB have same signaling pathways. And at the meantime, ETB can promote the activation of PLA2. The study also found that endothelins were widely associated with benign and malignant diseases.

    Objective To investigate the effect of the error correction method applied in the teaching of nursing training. Methods Two randomly selected classes were the same grade as the experimental group and the control group, the experimental group using error correction method of teaching, the control group using traditional teaching methods in order to operate the average assessment rate as the outstanding accomplishments and evaluation. Results The average student assessment results and operating performance was signiifcantly better than the control group superior (P<0.01). Conclusion error correction method of teaching can not only stimulate students' interest in learning, improve learning outcomes, but also can cultivate and train students to analyze problems and problem-solving skills, so that the overall quality of students can improved.

    Under the pressure of employment, taking part in the national graduates entrance examination is becoming the ifrst choice to postpone the employment pressure among the medical undergraduates, meanwhile, the clinical practice of these students is becoming increasingly neglected. Currently, the doctor-patient relationship is becoming tension, which results in a lot of difficulties to arrange and carry out the clinical training programs for medical students in teaching hospitals, those students could not reach the required clinical practices effects and qualities. Thus, it becomes a critical approach of how to strictly implement the internship program, conduct normative management and guide the medical undergraduates to overcome the interference in order to guarantee the teaching quality.
