Vascular calcification is the pathological basis of the occurrence and development of cardiovascular disease in patients with chronic kidney disease. Hyperphosphatemia is the most important risk factor for vascular calcification in patients with chronic kidney disease. It has been found that under high phosphorus conditions, microRNA(miRNA)plays a role in regulating vascular calcification by negatively regulating the expression of target cells after transcription to promote or inhibit calcification factors. Therefore, identifying the abnormal production of miRNA under hyperphosphatemia is helpful to identify epigenetic changes, which can be used as a target for the prevention and treatment of hyperphosphatemia induced vascular calcification. In this review, miRNA and their mechanisms related to high phosphate induced vascular calcification are reviewed.

Nonalcoholic fatty pancreatic disease(NAFPD)refers to fat deposition in pancreatic cells or fat replacement in pancreatic parenchymal exocrine glands with multiple factors, which is often closely associated with metabolic diseases. High lipid status in the pancreas may affect the function of islet β cells, lead to abnormal secretion of insulin or dysregulation of blood glucose level, and the occurrence of type 2 diabetes mellitus might be related to NAFPD. Imaging examinations can screen and quantify the content of pancreatic fat. This paper reviews the quantitative application of ultrasound, CT and MRI(IDEAL-IQI)in the quantitative application of pancreatic fat in type 2 diabetes mellitus and related clinical studies on NAFPD.The use of imaging for screening and quantification of NAFPD may provide new and relevant evidence for the diagnosis of subclinical diabetes.

Objective To investigate the clinical characteristics and risk factors of bronchopulmonary dysplasia(BPD)at different gestational ages, and provide targeted diagnosis and treatment guidance for children with BPD at different gestational ages. Methods The clinical data of 241 premature infants diagnosed as BPD in neonatal intensive care unit of Affiliated Hospital of Hebei University from February 2017 to February 2022 were collected and divided into three groups according to gestational age: gestational age <30 weeks group(105 cases), gestational age 30-32 weeks group(83 cases), and gestational age >32 weeks group(53 cases). The differences in general situation, maternal risk factors, treatment and complications among the three groups were compared. Results Among premature infants with BPD, there was no significant difference in gender and delivery mode among different gestational age groups(P>0.05), and the difference in birth weight was statistically significant(P<0.05). There was no significant difference in the scores of premature rupture of membranes, placental abnormalities and Apgar score among the maternal risk factors(P>0.05). There was significant difference in the scores of preeclampsia, gestational diabetes mellitus, prenatal hormone application and chorioamnionitis(P<0.05). There was no significant difference in the application rate of glucocorticoid and caffeine citrate after birth(P>0.05). There were significant differences in mechanical ventilation time, oxygen therapy time and hospitalization time(P<0.05). There was no significant difference in neonatal respiratory distress syndrome(NRDS)and sepsis among the complications(P>0.05). There were significant differences in pneumonia, intracranial hemorrhage(ICH)and periventricular leukomalacia(PVL)(P<0.05). Conclusion There are differences in clinical characteristics and risk factors among children with BPD of different gestational ages. Effective diagnosis and treatment should be carried out according to different gestational ages to avoid long-term mechanical ventilation and actively control the pulmonary infection that may be caused by mechanical ventilation, which can reduce the occurrence of the disease.

Objective To investigate the correlation of the incidence of anti-nuclear antibodies(ANAs)positivity and variants intramural hematoma(IMH). Methods 88 patients with a confirmed diagnosis of IMH admitted between November 2018 and July 2020 to Cardiovascular Surgery Department were included as the study group. The control group consisted of 88 age-matched chest pain patients without acute aortic syndrome confirmed by enhanced aortic CT. All patients were tested for ANAs in peripheral blood on the first day after admission. ANAs were detected by immunofluorescence assay. Titers > 1∶100 were regarded as positive for ANAs. Results Among the control subjects, 16(18.18%)were ANAs positive, 3(3.41%)were anti-neutrophil cytoplasmic antibodies(ANCA)positive, and among the study subjects, 48(54.55%)were ANAs positive, 12(13.64%)were ANCA positive.The difference between the two groups was statistically significant. Binary logistic regression analysis showed that atherosclerosis(OR=7.439, 95%CI=1.319-41.965, P=0.023), WBC(OR=2.907, 95%CI=1.812-4.663, P=0.001), D-Dimer(OR=2.942, 95%CI=1.406-6.154, P=0.004), ANAs(OR=13.618, 95% CI=2.437-76.085, P=0.003)and ANCA(OR=0.064, 95% CI=0.005-0.839, P=0.036)were independent risk factors for IMH. Conclusion The proportion of ANAs-positive patients in IMH group is higher than that in control group, and ANAs is an independent risk factor of IMH, which may become an important marker of IMH.

Mesenchymal stem cells(MSCs)are a kind of adult stem cells residing in a variety of tissues, such as bone marrow, adipose tissue and umbilical cord. With the developments of standard procedures and protocols for isolation, culturing and characterization of these cells, MSCs have emerged as an ideal seed cell for myocardial repair. When transplanted into a injured heart, MSCs have paracrine effects that nurture the injured area, prevent adverse cardiac remodeling, and mediate angiogenesis capabilities. MSCs have short survival time and risk of tumor formation, so they have not been widely used in clinic. In order to improve the therapeutic potential of MSCs and ensure its therapeutic safety, many methods such as gene modification, drug induction and bioengineering have moved from basic research to clinical practices, making MSCs become a high-quality resource cells for myocardial repair, bring new opportunity to myocardial infarction and end-stage heart failure.

This paper introduces YIN Sizhen's experience in diagnosis and treatment of gynecological diseases.YIN Sizhen was a famous gynecologist in Dingzhou of Baoding in the 1950s, ancestral Chinese medicine gynecology in three generations, practicing medicine for decades, with a wealth of clinical experience. This article is based on the oral contents of YIN Sizhen compiled by GUO Zhongyuan and ZHANG Fengchi of Hebei Academy of Traditional Chinese Medicine in 1960 to introduce his experience in gynecology. YIN Sizhen made an in-depth study of FU Qingzhu Nüke, payed attention to pulse-taking, realized that the focus of the book for the ups and downs of Yin and Yang. It is believed that most gynecological Re Zheng are based on deficiency of Yin and body fluid, rarely using antipyretic drugs with bitter cold; most of the gynecological Han Zheng are due to the insufficiency of Huo of Mingmen and Xinbao Luo and good at using of Yang warming drugs; good at Liqi and Huoxue drugs to regulate menstruation, as Yin and Yang, Qi and Xue are at peace, all syndromes can be cured.

Graduate students are an important talent reserve team in China. Their bodies and minds are relatively mature and they are in a period of active sexual activity, and they receive more free and flexible education. It is very necessary to understand the current situation of sexual health of graduate students. In this paper, 96 relevant literatures were collected from CNKI, Wanfang Data and VIP for visual analysis and content review. The research on sexual health of graduate students was not comprehensive, the students only knew the general knowledge of sex, and high-risk sexual behavior occured from time to time. The majority of scholars should pay more attention to the sexual health of postgraduates in their research work, and educational institutions such as colleges and universities should also pay attention to postgraduates' sex education.

Objective To provide the research basis for the next step of training needs investigation. Methods The item pool of advance care planning training needs assessment tool was initially constructed by literature analysis and data review. Then the English items were translated and adjusted according to the Brislin translation model, and the items were revised by Delphi expert consultation and pre-survey. Results After literature analysis and data review, 23 items were identified, involving three themes: basic knowledge, specialized knowledge and communication skills. After translation and back translation, the overall semantic evaluation of the scale was high, the semantic equivalence of the items was 78.6% to 100.0%, and the semantic equivalence of the scale was 82.6% to 100.0%. After two rounds of consultation, the enthusiasm of experts was 100%, the degree of expert authority was 0.87, the coordination coefficient of expert consultation was 0.271 and 0.353, the mean importance of each item ranged from 4.29 to 5.00, and the coefficient of variation ranged from 0.05 to 0.11. Among the 40 medical staff who evaluated the face validity of the scale, the recognition of each item of the scale was 87.5% to 100.0%. Conclusion The developed training needs assessment tool of advance care planning for medical workers is reasonable as a whole. The Chinese-translated items can provide a research basis for the next step of training needs investigation.

Objective To investigate and analyze the disinfection effect of high-frequency contact surfaces in bed units of patients with endotracheal intubation, so as to provide a basis for the development of environmental cleaning and disinfection. Methods The video review method was used to screen out the top 5 surfaces with the high-frequency contact surfaces in the bed units of 4 patients with endotracheal intubation. On the basis of disinfection every 12 hours for 5 consecutive days, the surfaces were sampled for bacterial cultured every 6 hours. The bacteria detection rate was used to evaluate the distribution of bacteria on the high-frequency contact surfaces and the disinfection effect at different sampling time points and time periods. Results The detection rates of bacteria on the surface of the five high-frequency contact objects were: 66.25% for the tracheal tube connector, 43.75% for the bed rail, 23.75% for the gastric tube connector, 6.25% for the syringe pump button and 2.50% for the monitor button(P<0.05), with statistics there was no statistical difference in the bacterial detection rate between different sampling time points and different working hours(P<0.05). The bacterial detection rate of 9 hours after cleaning and disinfection in different disinfection periods was significantly higher than that of 3 hours after disinfection, there was statistical difference(P<0.05). Conclusion The high-frequency contact surface of the tracheal intubation is the highest in the bronchial tube joint, the bed column and the gastric tube joint should be considered on the basis of cleaning and disinfection at intervals 12 h when developing an environmental cleaning disinfection plan, and the specific disinfection time needs further to explore.

Objective To explore and clarify the necessity and practicability of modular teaching reform in medical imaging technology and medical imaging equipment. Methods The students of medical imaging major of grade 2019 were divided into the experimental group and the control group, the teaching contents of medical imaging examination technology course and medical imaging equipment course were divided into modules. The experimental group used modular teaching method to teach, while the control group used traditional teaching mode to teach.The final examination scores of the two groups of students and their satisfaction with the teaching effect were analyzed. Results The results of final examination and teaching effect satisfaction survey of the experimental group were higher than those of the control group(P<0.05). Conclusion The courses of the medical imaging technology and medical imaging equipment, the implementation of the modular helps to build students' knowledge system, cultivate the students' subjective initiative, the training goal of medical image practical talents, further medical imaging technology and equipment, the practical problems in teaching.
