血尿素氮在监测危重症患者并发应激性溃疡出血中的应用

摘要: 目的 探究血尿素氮(blood urea nitrogen, BUN)等在监测危重症患者并发应激性溃疡出血中的变化特点及与出血量的相关性。方法 回顾收集唐山弘慈医院收治的危重症患者并发应激性溃疡出血的临床资料31例为出血组,并随机抽取同样例数未并发应激性溃疡出血的临床资料为对照组。监测2组患者入院24 h内、入院1周时、入院2周时的BUN、Cr、BUN/Cr、WBC计数、Hb含量,并对其变量进行差异性分析。结果 出血组入院1周时、入院2周时BUN、BUN/Cr、WBC均高于对照组和本组入院24 h内(P<0.001),Hb低于对照组和本组入院24 h内(P<0.001),2组Cr均无明显变化(P<0.05)。结论 BUN、BUN/Cr、WBC可作为早期发现危重症患者并发应激性溃疡出血及预测出血程度的简便而有效指标。

Abstract: Objective To explore the changes of blood urea nitrogen(BUN)in the monitoring of stress ulcer bleeding in critically ill patients and its correlation with the amount of bleeding. Methods Retrospective collection of critically ill patients in Tangshan Hongci Hospitals complicated with stress ulcer bleeding of clinical data(30 cases)and the same number of clinical data without stress ulcer bleeding were randomly selected as the experimental group and the control group, the clinical information of the monitoring of two groups of patients with BUN and creatinine(Cr), BUN/Cr ratio, white blood cell(WBC)count, hemoglobin(Hb), and the variable difference analysis were examined.Results In the bleeding group, BUN, BUN/Cr and WBC at 1 week and 2 weeks after admission were higher than those in the control group and this group at 24 hours after admission(P<0.001), Hb was lower than those in the control group and this group at 24 hours after admission(P<0.001).There was no significant change in Cr between the two groups(P<0.05).Conclusion BUN, BUN/Cr and WBC can be used as simple and effective indexes for early detection of stress ulcer bleeding and prediction of bleeding degree in critical patients.
