压力控制容量保证通气模式的临床应用进展

Pressure-controlled volume guaranteed ventilation in clinical application

摘要: 围手术期机械通气引起的肺损伤已成为全球广泛关注的问题,加强围手术期肺保护可以减少肺部并发症的发生,促进患者早日康复。术中机械通气策略与围术期肺损伤密切相关。压力控制容量保证通气(pressure-controlled volume guaranteed ventilation, PCV-VG)是一种新兴的通气模式,能够提供稳定的潮气量,且不受气道压力变化的影响。它通过确保气道压力保持在预设限制范围内,以实现适当的通气和氧供。现综合评估PCV-VG模式应用于围术期肺损伤后血流动力学、呼吸力学、炎症介质、氧合指数以及术后并发症等指标,并比较不同通气模式在特殊患者群体中的应用效果,为临床医生选择通气策略提供更可靠的依据。

Abstract: Perioperative mechanical ventilation-induced lung injury has become a globally recognized issue, and enhancing perioperative lung protection can significantly reduce the occurrence of pulmonary complications and promote early patient recovery. There have been studies indicating a close relationship between intraoperative mechanical ventilation strategies and perioperative lung injury. Pressure-Controlled Volume Guaranteed Ventilation(PCV-VG)is an emerging ventilation mode that can provide stable tidal volumes without being affected by changes in airway pressure. It achieves appropriate ventilation and oxygen supply by ensuring that the airway pressure remains within a predetermined limit. This review is to comprehensively evaluate the impact of PCV-VG mode on perioperative lung injury and outcomes, including hemodynamics, respiratory mechanics, inflammatory mediators, oxygenation indices, and postoperative complications, among other key factors. It also aims to compare the application effects of different ventilation modes in specific patient populations, providing more reliable evidence for clinicians in choosing ventilation strategies.
