Abstract: Objective To construct an extended care program of traditional Chinese medicine for hemiplegia patients with stroke and provide reference for the practice of extended care of traditional Chinese medicine for hemiplegia patients with stroke. Methods Based on the literature analysis, the Delphi method was used to conduct two rounds of correspondence with 20 experts. Results The questionnaire return rate for both rounds of expert correspondence was 100% and 95%, the finalized TCM continuity of care program for stroke patients with hemiplegia contained 6 level 1 entries, 20 level 2 entries and 52 level 3 entries, with authority coefficients of 0.860 and 0.900, and Kendall's W of 0.280 and 0.319, respectively, P<0.01. Conclusion The TCM- specific extended care program for stroke patients with hemiplegia constructed in this study has reliable and scientific results and is feasible.
