    Hypertension is a multi-factor disease with unknown etiology and serious complications. Due to the complexity of this disease, the pathogenesis of most patients with essential hypertension is still unknown. A large body of evidence shows that the purinoceptor signaling pathway plays a key role in a variety of diseases and constitutes a ubiquitous intercellular communication system. Almost all purinoceptors are expressed on the cells of the cardiovascular system. Some purine receptor subtypes has been confirmed in blood pressure regulation according to the existing research results and conjectures, but the knowledge is still limited. With the increasing understanding of the pathophysiology of purinoceptors, purine receptor compounds are also being developed for the treatment of hypertension. Therefore, the article summarizes the effects of purinoceptors on blood pressure and the latest research developments.

    Objective To screen key differential genes and identify the biological functions and signaling pathways involved in neutrophils from patients with primary antiphospholipid syndrome(APS)by bioinformatic methods. Methods RNA sequencing data of neutrophils from APS patients were downloaded from the GEO database, differentially expressed genes were screened by using the limma package in R language, and up- and down-regulated genes were subjected to gene ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis by clusterProfiler, respectively. Finally, protein-protein interaction network was constructed by STRING database and core genes in the network were calculated using cytoscape software. Results A total of 512 up-regulated genes and 473 down-regulated genes were obtained by limma package screening. Up-regulated genes were mainly involved in functions and pathways including regulation of innate immune response, neutrophil activation, inflammatory complex formation, etc. down-regulated genes were mainly involvedin functions and pathways including T cell activation, lymphocyte differentiation, lymphocyte activation, etc. IFIT1, IFI35, IFI3, CCL2, TLR8, FCGR1A were the core genes among the up-regulated genes. MYC, CD8A, FYN,CD28, LCK, GNB2L1 were the core genes among the down-regulated genes. Conclusion The core genes identified by bioinformatic methods may be the key targets for the immune effects of neutrophils in patients with primary antiphospholipid syndrome.

    With the acceleration of China's aging process, the incidence of chronic bone and joint diseases is increasing year by year, and one of the most effective treatments is total joint arthroplasty. At present, total joint arthroplasty has been widely used in clinic, but some patients still have complications. Because the preoperative factors of total joint arthroplasty can affect the incidence of postoperative complications, by improving the preoperative risk factors, we can provide the patients with artificial joint replacement with the best operation opportunity and reduce the incidence of surgical complications. This paper introduces the influence of common preoperative risk factors of total joint arthroplasty on the incidence of postoperative complications, hoping to provide some guidance for orthopedic surgeons in preventing postoperative complications.

    Brain metastases are intracranial malignant tumors with high incidence and poor prognosis, brain metastases from lung cancer are the most common in clinical practice. MR is the preferred imaging diagnosis method for intracranial tumors, and MRI imagingomics is gradually applied to the precise and individualized diagnosis and treatment of brain metastases. With the continuous in-depth research on the connection between genes and tumors, tumor diagnosis is extended to the direction of molecular genetics. Radiogenomics is a method of fusing imaging phenotypes with genes and molecules, using non-invasive and accurate methods to detect tumors. The molecular genetic changes of the disease have been widely used in tumor discovery, treatment guidance and prognosis prediction, etc. This article reviews the feasibility and research progress of MRI radiogenomics radio genomics in brain metastases.

    Acute pulmonary edema disease is a common clinical emergency and critical illness, and the disease develops rapidly, and it is necessary to have timely and effective diagnosis and treatment, otherwise it is easy to endanger the life of patients. Therefore, it is necessary to make early and correct diagnosis of acute pulmonary edema, especially to clarify its etiology, which is very important for the treatment and prognosis of patients.     Disturbance of calcium and phosphorus metabolism in chronic renal failure patients with long-term dialysis can cause compensatory hyperplasia of parathyroid gland and the increase of intact parathyroid hormone(iPTH)known as renal hyperparathyroidism(HPT)or secondary hyperparathyroidism(SHPT),clinical manifestations can include high parathyroidin, high calcium or hypocalcemia, and persistent high phosphorus. It can lead to diseases of skeletal system, neuropsychiatric system, blood system and cardio-cerebrovascular system, seriously affecting the quality of life and life span. The 2017 Global Guidelines for Kidney Disease recommend Chronic Kidney Disease(CKD)stage 3-5 complicated with severe hyperparathyroidism that does not respond to drug therapy, and parathyroidectomy is recommended. The problem of postoperative recurrence rate affects the further promotion of surgery. Although ultrasound and radionuclide scanning techniques have become more sophisticated in the past few years, it is still not possible to ensure complete preoperative localization of all parathyroids, especially those with less hyperplasia. The recurrence rate of subtotal resection and total resection plus autologous transplantation is higher than that of total resection, so total resection is more recommended for the prevention of recurrence. In addition, the improvement of preoperative localization, thymotomy, intraoperative parathyroid hormone monitoring and intraoperative nerve monitoring also contributed to the reduction of postoperative recurrence.

    China is facing a huge medical challenge from its aging problem. By 2050, China's population aged 65 and above will reach 400 million, of which the population aged 80 and above will reach 150 million. This paper reviews the evolution and current situation of population aging systematically through literature review, and analyzes some public health problems caused by China's aging population from the perspective of economic: underutilization of health service, the rapid rise of health costs, the huge demand for nursing service. It is suggested that we should strengthen health education for the elderly, expand nursing service, provide individual nursing care for them, further improve the medical security system and build a sustainable long-term care insurance system.

    By combing the development model and experience of community hospice care in China, this paper analyzes the main problems existing in the development of community hospice care, and puts forward reasonable suggestions for the development of community hospice care in China, such as increasing support and guarantee, strengthening the construction of talent team, establishing a correct concept of life and death so as to promote the faster and better development of domestic community hospice care services.

    The community home-based elderly care model is one of the elderly care models vigorously implemented in China. However, due to the particularity of the elderly group, there are some problems in the implementation of the community home-based elderly care model, such as the shortage of medical resources, the low degree of specialization and cooperation of service personnel, the poor interaction of multiple information, and the incomplete expression of the needs of the elderly. As a new way of organizational change, appreciative inquiry provides new ideas to solve the above problems. From the perspective of realizing the goal of appreciative inquiry, this paper puts forward relevant suggestions to improve the community home-based elderly care model in order to provide reference for improving the community home-based elderly care model.

    In recent years,with the acceleration of aging,the prevalence of benign prostatic hyperplasia(BPH)has shown an increasing trend,seriously affecting the quality of life of elderly men,and at the same time,it has brought an economic burden to the global medical care service system. In order to explore the current status of BPH management methods at home and abroad,an integrated analysis was carried out by searching relevant literature on BPH clinical progress management. At present,in order to delay and change the clinical progress of BPH patients,management methods combining clinical and non-clinical interventions have been adopted at home and abroad,and some results have been achieved,but there are still certain limitations. This paper analyzes and summarizes the current research status, and provides a reliable basis for the management of BPH clinical progress in the later stage.
