案例教学法在河北省住院医师规范化培训中的应用——基于教学双视角的调研

摘要: 案例教学法(case-based learning,CBL)在提升住院医师临床思维与实践能力方面具有重要作用。以河北省3所三甲医院住院医师规范化培训情况为研究对象,采用问卷调查和双主体视角,系统分析CBL在培训中的应用现状及存在的问题。案例教学法应用认可度高,效果显著,但案例库更新滞后、多学科协作不足、分层教学依赖学年制、教师培训体系不完善及评价机制单一等问题尚未得到有效解决。CBL在河北省住院医师规范化培训中的实际应用普遍存在问题,可构建动态更新的本土化多学科案例库,实施“能力评估+学年”双维分层递进教学策略,强化教师专业培训,完善多维度评价体系,推动信息化资源整合。为住院医师规范化培训质量的提升和教学模式的优化,提供一定的理论依据和实践指导。

Abstract: Case-based learning plays an important role in improving the clinical thinking and practical abilities of resident physicians. Taking standardized training resident physicians in three tertiary hospitals in Hebei province as the research object, this study systematically analyzes the application status and existing problems of CBL in the training using questionnaire surveys and a dual-subject perspective. The application of the case-based teaching method has high recognition and significant effects, but problems such as lagging updates of the case database, insufficient multidisciplinary collaboration, stratified teaching overly dependent on the academic year system, imperfect faculty training system, and a single evaluation mechanism have not been effectively resolved. The practical application of case-based teaching in standardized training of resident physicians in Hebei province generally has some problems. It is possible to construct a dynamically updated, localized multidisciplinary case database, implement a dual-dimensional stratified teaching strategy based on “competency assessment + academic year”, strengthen professional training for faculty, improve a multi-dimensional evaluation system, and promote the integration of information resources. This provides a certain theoretical basis and practical guidance for improving the quality of standardized training of resident physicians and optimizing teaching models.
