儿科规范化培训住院医师遗传代谢病教学情况分析

摘要: 目的 了解儿科规范化培训住院医师(住培医师)遗传代谢性疾病教学情况,旨在为儿科规范化培训教学改进提供依据。方法 使用问卷星对浙江大学医学院附属儿童医院106名规培住院医师进行问卷调查,了解现儿科规培教学中遗传代谢性疾病教学现况、存在问题及后期所需的改进措施、考核方式等。结果 86.79%住培医师认为遗传代谢病新筛意义重大,但仅0.94%表示熟悉流程。44.34%医师在规范化培训期间开始接触该类疾病,94.34%住培医师在规培期间遇到10种及以下该类疾病。95.28%住培医师认为需要系统学习该类疾病,50.94%认为学习非常困难。目前教学存在的最大问题为无规培轮转计划(68.87%)。教学改进方式方面,位列前三的分别为案例分析讨论(91.51%)、基础知识讲解(76.42%)、特殊面容识别教学(76.42%)。考核方式方面,医师更倾向于临床思维决策分析(64.15%)、案例分析(58.49%)及理论知识考核(54.72%)。结论 儿科住培医师对遗传代谢病认识不足。教学改进方向主要为提高住培医师兴趣、多元教学模式及丰富考核方式。

Abstract: Objective To understand the teaching of genetic and metabolic diseases in pediatricians' standardized training in pediatrics, so as to provide a basis for the teaching improvement of pediatric standardized training.Methods Using the questionnaire star, a questionnaire survey was conducted among 106 resident doctors in the Children's Hospital, Zhejiang University School of Medicine in order to understand the current teaching situation, existing problems, improvement measures and assessment methods of genetic metabolic diseases in the teaching of pediatrics. Results 86.79% of the residents thought that the new screening of genetic metabolic diseases was of great significance, but only 0.94% said they were familiar with the process. 44.34% of the trainees began to come into contact with this kind of disease during the standardized training, and 94.34% of the resident trainees encountered 10 or less such diseases during the regular training period. 95.28% of the students thought that systematic diseases were needed during regular training, and 50.94% thought that learning was very difficult. At present, the biggest problem in teaching was the random training rotation plan(68.87%). In terms of teaching improvement, the top three were case analysis and discussion(91.51%), basic knowledge explanation(76.42%)and special face recognition teaching(76.42%). In terms of assessment methods, students were more inclined to clinical thinking decision analysis(64.15%), case study(58.49%)and theoretical knowledge assessment(54.72%). Conclusion Resident pediatricians do not know enough about genetic metabolic diseases. The main direction of teaching improvement is to improve the interest of regular training doctors, multiple teaching models, and rich assessment methods.
