Hypertension is a multi-factor disease with unknown etiology and serious complications. Due to the complexity of this disease, the pathogenesis of most patients with essential hypertension is still unknown. A large body of evidence shows that the purinoceptor signaling pathway plays a key role in a variety of diseases and constitutes a ubiquitous intercellular communication system. Almost all purinoceptors are expressed on the cells of the cardiovascular system. Some purine receptor subtypes has been confirmed in blood pressure regulation according to the existing research results and conjectures, but the knowledge is still limited. With the increasing understanding of the pathophysiology of purinoceptors, purine receptor compounds are also being developed for the treatment of hypertension. Therefore, the article summarizes the effects of purinoceptors on blood pressure and the latest research developments.

Objective To screen key differential genes and identify the biological functions and signaling pathways involved in neutrophils from patients with primary antiphospholipid syndrome(APS)by bioinformatic methods. Methods RNA sequencing data of neutrophils from APS patients were downloaded from the GEO database, differentially expressed genes were screened by using the limma package in R language, and up- and down-regulated genes were subjected to gene ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis by clusterProfiler, respectively. Finally, protein-protein interaction network was constructed by STRING database and core genes in the network were calculated using cytoscape software. Results A total of 512 up-regulated genes and 473 down-regulated genes were obtained by limma package screening. Up-regulated genes were mainly involved in functions and pathways including regulation of innate immune response, neutrophil activation, inflammatory complex formation, etc. down-regulated genes were mainly involvedin functions and pathways including T cell activation, lymphocyte differentiation, lymphocyte activation, etc. IFIT1, IFI35, IFI3, CCL2, TLR8, FCGR1A were the core genes among the up-regulated genes. MYC, CD8A, FYN,CD28, LCK, GNB2L1 were the core genes among the down-regulated genes. Conclusion The core genes identified by bioinformatic methods may be the key targets for the immune effects of neutrophils in patients with primary antiphospholipid syndrome.

With the acceleration of China's aging process, the incidence of chronic bone and joint diseases is increasing year by year, and one of the most effective treatments is total joint arthroplasty. At present, total joint arthroplasty has been widely used in clinic, but some patients still have complications. Because the preoperative factors of total joint arthroplasty can affect the incidence of postoperative complications, by improving the preoperative risk factors, we can provide the patients with artificial joint replacement with the best operation opportunity and reduce the incidence of surgical complications. This paper introduces the influence of common preoperative risk factors of total joint arthroplasty on the incidence of postoperative complications, hoping to provide some guidance for orthopedic surgeons in preventing postoperative complications.

Disturbance of calcium and phosphorus metabolism in chronic renal failure patients with long-term dialysis can cause compensatory hyperplasia of parathyroid gland and the increase of intact parathyroid hormone(iPTH)known as renal hyperparathyroidism(HPT)or secondary hyperparathyroidism(SHPT),clinical manifestations can include high parathyroidin, high calcium or hypocalcemia, and persistent high phosphorus. It can lead to diseases of skeletal system, neuropsychiatric system, blood system and cardio-cerebrovascular system, seriously affecting the quality of life and life span. The 2017 Global Guidelines for Kidney Disease recommend Chronic Kidney Disease(CKD)stage 3-5 complicated with severe hyperparathyroidism that does not respond to drug therapy, and parathyroidectomy is recommended. The problem of postoperative recurrence rate affects the further promotion of surgery. Although ultrasound and radionuclide scanning techniques have become more sophisticated in the past few years, it is still not possible to ensure complete preoperative localization of all parathyroids, especially those with less hyperplasia. The recurrence rate of subtotal resection and total resection plus autologous transplantation is higher than that of total resection, so total resection is more recommended for the prevention of recurrence. In addition, the improvement of preoperative localization, thymotomy, intraoperative parathyroid hormone monitoring and intraoperative nerve monitoring also contributed to the reduction of postoperative recurrence.

The community home-based elderly care model is one of the elderly care models vigorously implemented in China. However, due to the particularity of the elderly group, there are some problems in the implementation of the community home-based elderly care model, such as the shortage of medical resources, the low degree of specialization and cooperation of service personnel, the poor interaction of multiple information, and the incomplete expression of the needs of the elderly. As a new way of organizational change, appreciative inquiry provides new ideas to solve the above problems. From the perspective of realizing the goal of appreciative inquiry, this paper puts forward relevant suggestions to improve the community home-based elderly care model in order to provide reference for improving the community home-based elderly care model.

Ischemic stroke is one of the main causes of death and disability worldwide. Nuclear factor E2-related factor 2(Nrf2)is an important transcription factor that regulates the balance of the body's redox reaction. It participates in resisting endogenous or exogenous stress by coordinating a variety of cytoprotective genes. The Nrf2 signaling pathway not only strictly regulates the body's redox homeostasis, but also participates in the regulation of multiple processes in the body metabolism. Therefore, targeting Nrf2 has become a potential strategy for the prevention and treatment of central nervous system diseases including ischemic stroke. In this review, we first introduced the structural features of Nrf2 and Kelch-like ECH-related protein 1(Keap1), and briefly described the activation and regulation of the Nrf2 pathway. Next, we discussed the pathological and physiological mechanism, upstream regulators and downstream targets of the Nrf2 pathway. In this context, we extended the discussion to the role of Nrf2 after ischemic stroke and explored potential therapeutic directions.

Vasculogenic mimicry is a special model of angiogenesis. This structure is composed of tumor cells instead of traditional endothelial cells, and provides nutrition for tumor growth, invasion and metastasis. In recent years, it has been found that vasculogenic mimicry is related to the prognosis, invasion, metastasis and drug resistance of many kinds of malignant tumors. It is generally believed that vasculogenic mimicry and angiogenesis depend on some common mechanisms. And some people believe that the formation of vasculogenic mimicry is one of the reasons leading to the development of resistance to anti-angiogenic drugs. The mechanism of vasculogenic mimicry in breast cancer, including related signaling pathways, as well as tumor stem cells, epithelial mesenchymal and anti angiogenesis therapy, are reviewed. The significance and future direction of vasculogenic mimicry as a therapeutic target for breast cancer will be prospected.

Myopia is a common disease in the population, and its incidence is increasing year by year, especially in children and adolescents, seriously affecting their physical and mental health. Binocular vision refers to that two eyes see at the same time, while external objects fall on the retina, forming retinal images, nerve excitatory transmission go to the brain higher center, so that the visual signals of two eyes merge into a single perceptual object. When viewing stereoscopic objects, due to the difference of normal pupil distance and the angle of faxation, the images formed on the retina of the two eyes are not completely symmetrical and have certain horizontal difference. This phenomenon is called Binocular Parallax, which gives us stereoscopic perception. Many studies have shown that binocular vision functions such as accommodation, convergence, stereoscopic vision and the development of myopia interact. The purpose of this article is to review the developments of binocular vision function and myopia in order to provide reference for the prevention and control of myopia to choose effective and appropriate programs.

Kümmell's disease(KD)is a delayed form of vertebral ischemic necrosis after minor trauma. KD shows delayed vertebral compression fracture, and the most common lesion is at the junction of thoracolumbar spine. Patients with KD often have a history of minor trauma. Although the postoperative pain disappears, the symptoms often relapse and worsen, and kyphosis deformity occurs many years later. The diagnosis of KD depends not only on imaging technology, but also on the comprehensive analysis of patients' medical history and clinical symptoms. Intraspinal fracture caused by trauma is considered to be the most important feature in the diagnosis of KD. Patients with KD can adopt different treatment schemes according to whether there is nerve defect. Patients with KD without nerve defect can be treated with conservative treatment, percutaneous vertebroplasty(PVP), percutaneous kyphoplasty(PKP)and other methods. However, the above methods are often accompanied by leakage of bone cement. With the development of modern medicine and technology, in order to reduce the leakage of bone cement, clinicians have made a series of improvements to the operation on the basis of PVP, PKP, anterior reconstruction and posterior osteotomy, which effectively reduces the occurrence of complications and improves the quality of life of patients.
