    Biomechanics study on the femoral neck with side hole hollow nail combined with bone cement

    Objective To study the healing effects by applying femoral neck with side hole hollow nail combined with bone cement for the promotion of fracture. Methods 58 adult femoral neck specimens were seletced based on the maximum axial pull output (F-Max) in experimental group, 12 femoral neck in bored after the hollow screw directly into the center of the axial, the other 12 femoral neck in the center of the axial drilling will polymethacrylate again after infusion of bone cement into side holes with hollow screw. To pull out the experiment, the hollow nail pulling power of the maximum axial and screw in the process of cycle resistance to bending experiment in loose could withstand the maximum load were determined. Results In PMMA bone cement injection hole hollow nail was significantly higher than that of the maximum axial force ,which was higher than pure hollow nail maximum axial pull output, the difference was statistically significant (P<0.05). Conclusion For hip osteoporosis fracture, the side holes by using methyl methacrylate hollow nail fixed the fracture end, can significantly reduce the risk of femoral head loose, falling off, and improve the stability of the fixed the fracture end, accelerate fracture healing.

    Objective To observe how magnetic field interference effect on nerve scar formation after nerve injury in rats. Methods 80 SD rats were implemented left sciatic nerve entrapment surgery, with nerve damaged. After surgery, the rats were randomly divided into four groups: 1 control group and 3 experimental groups. Experimental groups were respectively given 0.2 mT’s, 0.6 mT’s, and 1.5 mT’s intensity magnetic therapy, 60 min/d. Sciatic nerve function index measurement (SFI) was tested in 21 days. After drawn, the samples were applied, type collagen fibers immunohistochemical staining, transmission electron microscope and experimental data statistics processing were performed. Results Compared with the control group, the experimental group was closer to normal in nerve function index, with less, type collagen fibers hyperplasia in Nerve lesions, thicker myelin sheath and line rules walking.Conclusion Magnetic field interference can significantly inhibit scar formation, promote nerve regeneration and functional recovery.

    Objective To investigate the reasonable delivery way during different periods (37-41+6 weeks) of uterogestation accompanying with oligohydramnios. Methods From January 2010 to June 2013, 142 full-term gravidas were selected as research objects, which were diagnosed as oligohydramnios (5+6 weeks), the interim group (39-40+6 weeks) and the advanced group (41-41+6,/sup> weeks), were divided. Full-term gravidas with normal amniotic fluid diagnosed by hameochronous ultrasonography and without other high risk factors were randomly selected as control group. The success rate of vaginal delivery, fetal distress, emergency caesarean section rate and asphyxia neonatorum were observed respectively. Results (1) The success rate of vaginal delivery showed a significant tendency of interim group>advanced group>early group, but the significance between the research group and control group was not obvious. (2) Compared with the control group, the failure rate of odinopoeia showed a tendency of early group>interim group=advanced group, in which the significance between the early group and control group was not obvious, while the opposite was true among the interim group, advanced group and control group. (3) The emergency caesarean section rate showed a significant tendency of advanced group>early group>interim group, but the significance was not obvious between the three research groups and control group. (4) The significance of non-stress test adiaphoria was not obvious between the research groups and control group. (5) The significance of mild asphyxia neonatorum between the early group and control group was not obvious, while the opposite was true among the interim group, advanced group and control group. Conclusions Oligohydramnios occurred among the interim and advanced cases. By this time, low doses of misoprostol could be used to promote the cervical ripening and labor induction in case the fetus possesses a fine fetal heart. However, the vaginal delivery indication should be well controlled, and during the whole delivery process, the fetus’s condition must be closely monitored in order to provide appropriate measures. During the early period, vaginal delivery should be carefully adopted if the oligohydramnios occurs. Fluid infusion could be used only when the fetus possesses a fine fetal heart. On the contrary, if the fetal distress occurs, the indications for cesarean section should be relaxed.

    Objective To investigate the clinical characteristics of PVT in order to find out effective approaches on its diagnosis and treatment. Methods The clinical data of 135 consecutive patients with non-neoplastic liver cirrhosis who had undergone splenectomy from January 2010 to January 2013 in our institution were collected. Clinical and surgical characteristics of the patients who developed PVT postoperatively and those who did not develop PVT were compared. Results 135 patients after splenectomy in 16 cases (11.85%) occurred in PVT, all patients were confirmed by color Doppler ultrasound or contrast-enhanced CT examination. The comparison of preoperative, postoperative width of portal vein(1.53±0.19) cm vs (0.91±0.19) cm, splenic vein diameter (1.36±0.27) cm vs (0.75±0.19) cm, portal vein blood flow velocity (10.33±1.98) cm/s vs (13.65±2.31) cm/s were thrombus group than in non portal thrombosis group had significant difference. In 15 cases with thrombus dissolving anticoagulant therapy in whole or in part, of which 5 cases underwent percutaneous portal vein puncture and catheterization thrombolysis, followed up for 1 years without recurrence. Conclusion Preoperative portal vein diameter and blood flow velocity in predicting postoperative portal vein thrombosis has the function of guidance. Once PVT is diagnosed, timely anticoagulation therapy, interventional therapy should be considered as a good choice.

    Objective To explore the diagnostic value of transesophageal echocardiography (TEE) in the left atrial and left atrial appendage thrombus before atrial fibrillation radiofreguency ablation treatment. Methods 52 cases of patients with atrial fibrillation received check of transthoracic echocardiography (TTE)and TEE, before radiofreguency ablation treatment, the situation of left atrial and left atrial appendage thrombus were summarized. Results In 52cases patients with TTE, left atrial thrombus were found in 4 case(7.69%), left atrial appendage thrombus in 3 case (5.77%), The false positive in 1of 3 cases, left atrial spontaneous echo contras (SEC)in 2 case (3.85%); In 52cases patients with TEE, left atrial thrombus were found in 8 cases (15.38%), left atrial appendage thrombus in 9(17.31%), left atrial SEC in 8(15.38%), left atrial appendage SEC in 6(11.54%). Every indice had statistically significantly in all patients between TEE and TTE.The Uitrasonic image of left atrial or left atrial appendage thrombus showed abnormal clumps echo attached to the wall. Swirling or cloudy signs which were explored in left atrium or left atrial appendage were SEC, It is hypercoagulable state. Conclusion The detection rate of left atrial and left arterial appendage thrombus is increasesed by TEE. It is a effective method before atrial fibrillation radiofreguency ablation treatment.

    Objective To investigate the characteristics of carotid atherosclerotic lesions in patients with hyperthyroidism. Methods 72 patients with hyperthyroidism ( hyperthyroidism group) and 63 cases of healthy volunteers ( control group) were investigated, the changes of CIMT, ET, NO, CRP were observed. Results CIMT in hyperthyroidism group with above 45 years old was higher than those of the hyperthyroidism group and control group with Less than 45 years old. The levels of ET, NO, CRP in hyperthyroidism group were higher than those of the control group. Conclusion Hyperthyroidism is associated with endothelial damage and increase the risk of atherosclerosis

    Objective To investigate the relationship of JAK2V617F mutation and cerebrovascular events in patients with myeloproliferative neoplasms(MPN). Methods The JAK2V617F mutation of 64 patients with MPN was detected. We performed a retrospective study on the incidence of cerebrovascular events(cerebral infarction, transient ischemic attack, cerebral hemorrhage, subarachnoid hemorrhage) in 64 patients with MPN. Statistical analysis was applied on the incidence rate of cerebrovascular events between JAK2V617F mutation positive group and negative group and between groups of different diseases with JAK2V617F mutation. Results Of 64 patients with MPN, JAK2V617F mutation was detected in 47 cases (73.4%). There were a total of 38(59.4%) cerebrovascular events. The incidence rate of cerebrovascular events in JAK2V617F mutation positive and negative patients were 68.1% and 35.3% respectively, P=0.018. The incidence rate between PV and ET patients with JAK2V617F mutation were statistically non-significant. Conclusion The risk of cerebrovascular events in MPN patients with JAK2V617F mutation may be higher.

    Objective To explore the treating effects of community comprehensive management combined with Paroxetine in relieving depression and anxiety of patients with digestive malignancy. Methords 89 digestive malignancy patients with depression and anxiety were researched in the Community health center areas of medium-sized cities, which was treated with Paroxetine combined with community comprehensive management. The SCL-90 SAS SDS and FACT-G were analyzed by statistical method before and after treatment between 2011.2 and 2014.2. Results SCL-90 scores indicated that patients in the survey community had depression, anxiety before the treatment. And the SCL-90 scores were improved after the treatment, there was statistically differences (P<0.05). Comparing SAS SDS FACT-G scores for estimating depression and anxiety degrees of patients, they were decreased obviously after the treatment (P<0.05). Conclusion Community comprehensive management with Paroxetine is an effective treatment to relieve depression and anxiety status, and can improve the life quality of digestive malignancy patients.

    Objective To observe the clinical efficacy of conventional therapy with auricular point sticking in treating chronic prostatitis. Methods Eighty-four patients with chronic prostatitis were randomized into treatment group and control group (42 in each group). The treatment group was intervened by oral conventional medical therapy (such as antibiotics and/or Tamsulosin) with auricular point sticking, and the control group was given by oral antibiotics and/or Tamsulosin. Therapeutic efficacies were compared between the groups. Results The total effective rate was 85.72% in the treatment group versus 52.17% in the control group, and the difference was statistically significant (P<0.01). Conclusion The clinical efficacy of conventional therapy with auricular point sticking in treating chronic prostatitis is definite and significant, which can improve the whole quality of life and the psychological barrier of the patient.

    Objective To discuss the clinical effect of irbesartan combined with amiodarone in rebuilding and maintaining sinus rhythm in patients with paroxysmal atrial fibrillation. Methods 100 patients with paroxysmal atrial fibrillation randomly were divided into control group (n=48) and therapeutic group (n=52).Both groups received amiodarone, therapeutic group received irbesartan additionally.All patients were follwed up for 1year. The diameter of left atrium (LAD), left ventricular ejection fraction (LVEF) and the level of hs-CRP were counted, The rate of successful sinus rhythm rebuilding and its maintenance were also observed. Results Compared with the control group, the LAD decreased and LVEF improved in therapeutic group, the rate of successful sinus rhythm and its maintenance were significantly increased. The level of hs-CRP in both groups was lower, especially in the therapeutic group. Conclusion Irbesartan combined with amiodarone can inhibit atrial remodeling obviously, improve heart function, increase the rate of in the rebuilding of sinus rhythm and decrease its recurrence rate significantly, which is more effective than amiodarone alone.

    Objective To investigate the application results and adverse reactions of sufentanil combined with flurbiprofen in relieving postoperative analgesia in patients with lower limb fractures. Methods 60 patients with lower extremity fractures underwent open reduction and internal fixation (ORIF) were selected in our hospital, according to a random number table they were divided into two groups, 30 cases in each group. The patients of the two groups were using disposable constant speed analgesia pump after operation, the observation group applying analgesia pump recipe sufentanil combined with flurbiprofen, the control group using sufentanil. Record pain visual analog scale and adverse reactions after operation for 2 h, 4 h, 8 h, 12 h, 24 h. Results There was not statistically significance considering analgesic effects in the 2 groups and the difference of point pain visual analog scale groups after operation (P> 0.05). The complications such as nausea, vomiting, respiratory depression, bradycardia, sedation and other adverse reactions in the controls were significantly higher than in the observation group (P <0.05 or 0.01). Conclusion Sufentanil combined with flurbiprofen is a safe and ideal analgesia medicine used in patients with lower limb fractures.

    Objective To investigate the effect of preoperative application of phloroglucin in adolescent painless induced abortion. Methods Between January, 2011, and May, 2014, 52 adolescent pregnant women requiring induced abortion in our out-patient department, aged 14 to 19 years old, were randomly divided into two groups. 27 patients in observation group and 25 in control group. Before the start of the operation for 10 min, the patients in the observation group were given 80 mg phloroglucin intravenously, then 1.50-2.50 mg/kg propofol intravenuously until the patients lost consciousness, and then received the induced abortion. Patients in control group were only treated with propofol with the same dose for painless induced abortion. Observation index included uterus flabby degree, operation time, analgesic effect and the occurrence of artificial abortion syndrome. Results The effective rate of analgesia was 96.29% in the observation group, and 88.00% in control group. There were significant differences between two groups ( P<0.05). There was also significant difference between two groups in the uterus flabby degree, operation time and the occurrence of induced abortion syndrome control group ( P<0.05). Patients in the observation group had no complications, while one patient in the control group appeared complication of uterine perforation because of tight Uterus. Conclusion The application of phloroglucin intravenously in adolescent painless induced abortion, can significantly enhance the analgesic effect, effectively improve the uterus flabby degree, shorten the operation time, reduce the incidence of induced abortion syndrome. Therefore it can be used to reduce the occurrence of complications, which is worthy to be distributed in clinical practice.

    Objective To observe the efficacy and safety of Ginkgo biloba injection in treating blood hypercoagulable state of patients with advanced lung cancer. Methods 109 cases with advanced lung cancer in the Affiliated Hospital of Hebei University were divided into experimental group (50 cases) and control group (59 cases) between 2010.10 to 2014.10. The patients of the control group were treated only with systemic chemotherapy, those of the experimental group were also treated with chemotherapy combined with Ginkgo biloba injection. Fib, D-D levels and curative effects were compared after 2 courses of treatment, which one period of treatment was 7 days. Results The improvement rate of experimental group was 81.36% (48/59), and 26% (13/50) in the control group. Inefficiency in the experimental group and control group was 8.47% (5/59) and 32% (16/50), and the stability factor was 10.17% (6/59) and 42% (21/50) ( both P<0.05). Comparing two groups, the Fib and D-D levels did not have any significant difference before treatment (P>0.05). However, the two levels of the experimental group were lower than those of the control group after treatment, and the difference was significant (P<0.05). The difference about the Fib and D-D levels before and after treatment between the experimental group and the control group was significant (P<0.05), showing that the curative effect of the experimental group was better than that of the control group.Experimental group did not appear obvious adverse reactions. Conclusion Ginkgo biloba injection can improve blood hypercoagulable state of patients with lung cancer, which is effective and safety.

    Objective To explore four-handed operation technology in orthodontic referral process. Methods 50 patients with orthodontic referral were randomly divided into experimental group and control group, the operation of both groups were performed by the same senior doctors. USE West Baltic non-mixing type enamel bonding resin, and brackets selection of domestic Ossur MBT orthodontic brackets were used in the operation. The patients of the control group were applied with non-four hands technique, the entire operation process was assisted by a nurse ,a physician completed the operation. The experimental group received four hand manipulation techniques, in the course of treatment a full-time nurse assisted the physician in practices, conducted strict implementation of disinfection system, complete equipment delivery, every wet assistance, public education and other auxiliary work. The comparison of the operating time, the terms of the bracket drop rate after operation for 30 days and the degrees of the patients’ comfort between the two groups were compared. Results The differences in comfort of the patients between the experimental group and the control group were not statistically significant, the operating time, the terms of the bracket drop rate in the experimental group after operation for 30 days were better than that of the control group. Conclusion Four-handed technique applied can obviously improve the success rate of bracket bonding, reduce the intensity of the doctor's work and shorten the treating time and there is not significant difference in terms of patients’ comfort between the two groups.

    Objective To investigate the effects of the community management applications on blood glucose of patients with type II diabetes and provide the basis for the improving the development of community diabetes management model. Methods 120 patients with type 2 diabetes were randomly divided into experimental group and control group, 60 cases in each group. The given medicines of the 2 groups were the same .However, the experimental group received community follow-up, health education, lifestyle guide, medication management and but the control group were not received. The results of FPG, 2hPG, HbA1c of two groups were compared. Results After 1 year of the management, FPG, 2hPG, HbA1c were significantly reduced in the patients of the experimental group, with statistical significance (P<0.01) and the difference between the baseline value; after 1 year the levels of FPG, 2hPG, HbA1c in the patients of the control group were significantly increased compared with that of the experimental group with baseline values. The differences was statistically significant (P<0.01). 1 years later, FPG 2hPG, HbA1c of the patients of the two groups had a statistically significant difference (P<0.05). Conclusion The community management of diabetes prevention and treatment of type 2 diabetes has a positive effect, which can significantly lower blood sugar levels. Strengthening community management of diabetes is an important measure to effectively control the development of diabetes.

    Objective To guide the formulation of prevention focus of disease control and get reasonable configuration of the limited health resources through the evaluation of main malignant tumors burden in Baoding urban residents of Hebei province in 2009. Methods This paper evaluated Baoding urban resident's malignant tumor burden by analyzing the incidence of malignant tumors and the death cases. Besides, it used the method of the disability adjusted life years(DALY), which was calculated by the early loss of life years (YLL) death and disability loss of life years (YLD). Results In 2009, the incidence in urban residents of Baoding was 232.44/100000 and the cancer mortality was 75.58/100000, the top 5 burden of malignant tumor were bronchial and lung cancer, liver cancer, gastric cancer, esophageal cancer and colorectal cancer, the DALY for per thousand were 4.8, 1.9, 1.2, 1.1 and 0.9 years respectively, the malignant tumor in 2009 in Baoding city caused the loss of 12.8 life years per thousand people, the burden of malignant tumor mainly occurred in the people over 45 years old. Conclusion In the urban residents of Baoding, the burden of bronchial and lung cancer, liver cancer, gastric cancer, esophageal cancer and colorectal cancer is heavy and mainly concentrated in the people over 45 years old.

    Objective To explore the syncope causes and its nursing countermeasures in college students conducted physical examination through venous blood collection. Methods The causes were analyzed in 11 cases of college students registered in 2013 occurred syncope during physical examination through venous blood collection, and the corresponding nursing measures were put forward. The methods of physical check-up work in 2014 was reformed. The incidence of 2013 and 2014 syncope causes of the examinated students were compared. Results Our results indicated that there were a statistically significant differences between 2014 and 2013 in the examined students (P<0.01). Conclusion Through the analysis of the syncope causes of the college students and taking corresponding measures can effectively reduce the incidence of syncope in college students.

    Objective To investigate the causes and preventive measures in patients with deep vein thrombosis combined with stroke. Methods A retrospective study of 22 cases with deep vein thrombosis combined with stroke from July 2013 to July 2014 in the Affiliated Hospital of North China University was conducted . Their general information, treatment, care measures and its etiology were determined. Results After treatment, 16 cases were cured, five cases improved, one case was dead, the total effective rate was about 95.45%. Conclusion The main causes for deep vein thrombosis combined with stroke were old, brake, blood coagulation, vascular wall injury, taking early prevention of these risk factors can be avoided the stroke with DVT, once the blood clots formed, applying anticoagulation, thrombolytic therapy , with personalized care and measures can reduce the degree of disability and improve the quality of life to patients.

    The rise of blood leukocyte count can reflect the prognosis of patients with acute myocardial infarction (AMI). This paper summarized from the following aspects: the mechanism of elevated white blood cell count in patients with AMI, white blood cell count and AMI reperfusion therapy, white blood cell count and AMI infarction area, the relationship between neutrophil counts and the prognosis of AMI, the relationship between neutrophil/lymphocyte ratio and the prognosis of AMI, lymphocyte count and the prognosis of AMI, monocytes and the prognosis of AMI. Research shows that in AMI early detection of peripheral blood leukocyte count and classification, reducing the white cell number and function timely have a certain clinical significance to prevent the infarction area expansion and reduce the occurrence of complications and reduce the mortality.

    Global rise in male infertility as a result of falling sperm count and quality has been pointed out by many investigations. Therefore, it is essential to understand the molecular mechanism of spermatogenesis and its regulation. Spermatogenesis is characterized by three phases: mitosis, meiosis and spermiogenesiss, which are strictly regulated by phase-specific gene expression that is controlled by myriads of regulatory non-coding RNAs. Rapid advancement in genome mining technologies has identified role of regulatory non-coding RNAs including microRNA(miRNA), small-interfering RNA(siRNA), PIWI-interacting RNA(piRNA) and long non-coding RNA(lncRNA) as controllers of gene expression at transcriptional as well as post-transcriptional level in different biological context and disease processes. Here, we discuss the recent advances in our understanding about the involvement of these transcripts in spermatogenesis. In addition, we review here the relationship between their abnormal experssion and associated diseases.

    Objective To study the current status of hospice care education in Chinese medical colleges and put forward some suggestions for the future of hospice care education in our country through investigation of hospice care education in six medical colleges and universities in Shandong province. Methods Using cluster sampling, 600 on-campus college students in Shandong were selected and they were investigated with questionnaires. Results Nearly 86.6% of the students had hospice care related courses and 13.4% of the students did not offer hospice care course. Teaching methods mainly based on classroom instruction and performance evaluation for a closed book exam, thus students were generally lack of hospice care skills. Conclusion Hospice care education in our country is still to be further popularized and improved.
